Obtaining Records / Filing Complaints

If you wish to obtain your psychological records simply call Dr.  Dayton or go on your portal and request them.  

Additionally, if you wish to submit a written complaint about Dr. Dayton  you can do so by contacting the U.S. Department of Health and Human Services or the Texas Behavioral Health Executive Council (TBHEC) at  bhec.texas.gov or file  at email address: complaints@bhec.texas.gov or call their phone # at 512-305-7700 if you are in Texas  or  you can file a consumer complaint with the office of the Attorney General’s  Consumer Protection webpage at https://consumerprotection.texasattorneygeneral.gov/consumercomplaintportal/s/#:~:text=Here%20you%20can%20learn%20how,New%20Complaint’%20to%20get%20started

In Massachusetts you can file a complaint with the Commonwealth of of Massachusetts Division of Psychologists email pyboard@mass.gov or call (617) 701-8782 if you were a client in MA.  For New Hampshire clients, Dr Dayton is also licensed by  the New Hampshire Psychology Board (License #1562)which can be reached for complaints at Psychology@oplc.nh.gov or by calling (603) 271-2152.  If you reside outside those states you can contact the state psychology board in your state.  Your therapist will provide you with the address to file your complaint, and no retaliatory action will be take.
